The Grant County Health District is requesting proposals from qualified firms or individuals to develop the Grant County County community health assessment and the health improvement plan for Grant County County. The purpose of the Grant County Health District and improvement plan is to provide evidence-based data and information on factors that influence the health of Grant County County residents.
The Grant County Health District’s vision is to develop a community-wide effort to improve the health status of all Grant County County residents through prevention strategies, partnerships, education, and advocacy efforts.
